Three populations make up the national funnel, and they must never be mixed: applicants (unique people who submitted applications), acceptees (applicants accepted to at least one school), and matriculants (accepted applicants who enrolled). The acceptance rate is acceptees ÷ applicants; the matriculation rate is matriculants ÷ applicants. They are different numbers — a school's "acceptance rate" and "matriculation rate" answer different questions.
First-time applicants are people applying for the first time; re-applicants have applied in a prior cycle. Committees track the first-time share because it signals pipeline health: a rising first-time share with rising totals means genuine new interest, not the same people cycling back.
The 2021–2022 cycle was the pandemic peak for applicants; the pool then contracted for two cycles before rebounding in 2025. Matriculants, by contrast, have grown almost every year — enrollment capacity keeps expanding while applications fluctuate.

First-time applicants surged 21.2% in the 2021–2022 pandemic cycle, fell 17.2% the next year, drifted down one more year, then rebounded: +2.3% in 2024–2025 and +8.4% in 2025–2026. In 2025, first-time applicants were 76.5% of the total applicant pool, while re-applicants declined 3.6%.

The AAMC's MCAT/GPA acceptance grid shows how sharply the probability of being accepted to at least one school rises with GPA. Across the three cycles 2021–2022 through 2023–2024, the overall acceptance rate (all GPA bands, computed from the grid's applicant and acceptee counts) was about 42% — roughly two in five applicants were accepted somewhere.

The matriculant pool is academically stronger than the applicant pool — as it must be, given that acceptance rises with GPA and MCAT. For the 2023–2024 cycle:
